STEP 1: Check this calendar for availability
STEP 2: Fill out the Request Form.
Notes:
- Reservations are to be made 3 DAYS BEFORE THE SCHEDULE.
- Rename the pdf file to this format, YYYY-MM-XX_SURNAME_FIRST NAME.
YYYY- Year
MM- Month (Example: For January, type “01”)
XX- Date today
- The Request to Use Analytical Equipment is a pdf fillable form. Download the file, open using any PDF reader, and fill in the necessary information.
- Please include the name of the Research Project (or the agency to be billed) as well as the name of the requestor in the “NAME” box.
- Upload the accomplished pdf fillable form. Upload the file as the editable format. This is for CEAL documentation purposes.
- DO NOT PRINT AND SCAN THE FILE as this will defeat the purpose of the pdf fillable form.
STEP 3: Upload the accomplished form in this Google Form.

3.) Upload the accomplished pdf fillable form. Upload the file as the editable format. This is for CEAL documentation purposes.
4.) DO NOT PRINT AND SCAN THE FILE as this will defeat the purpose of the pdf fillable form.
5.) Sample turnaround time is 1-2 weeks upon receipt of proof of payment.
6.) Please include the type of analysis in the sample description form.
